Adherence Initiative for Medication Management (AIM)

PACE practitioners know that medication management is a pivotal aspect of all-inclusive care. But often providing excellent participant care requires an understanding of factors beyond health status. To reduce patient risk, we must understand adherence behaviors. The CareKinesis Adherence Initiative for Medication Management (AIM) accomplishes three goals:

  1. Establishes baseline medication adherence scores for PACE participants to gain understanding of participants’ medication habits.
  2. Assists with selection of adherence interventions.
  3. Trends intervention outcomes with adherence re-assessments.

CareKinesis has created a brief assessment tool for use in the PACE population, and is collaborating with PACE organizations to assist with participant medication risk mitigation. This tool provides actionable insight about attitudes/behaviors and barriers to adherence.
